CBP Declaration Form 6059B provides information to U.S. Customs and Borders Protection about the traveler, including where they have been before coming to the United States and what they plan to bring into the country. You can fill out one form if you’re traveling with your immediate family.
CBP Form 6059B Components
You can fill out the Customs Declaration form with your name, contact information, and passport number. It also includes space for information about countries visited before arrival. The OMB control number is 1651-0009
Step-by-step instructions on how to complete the CBP Form 6059B
You must provide the following information for CBP Form 6059B Customs Declaration:
- First name, middle name
- Birth date
- The number of family members who are traveling with you
- Address in the United States
- Country of passport issuance
- Passport number
- Country of residence
- Before the arrival of the United States, countries were visited
- Name of the airline/flight number
- It doesn’t matter if your trip was for business.
- Declare whether you are bringing:
- Fruits, vegetables, plants, seeds, food, insects
- Meats, animals, and animal/wildlife products
- Snails, cell cultures, disease agents
- Have you ever worked in soil or on a farm/ranch/pasture?
- You may have been within proximity to livestock
- You can carry currency and monetary instruments above $10,000, regardless of whether you have them.
- If you are selling commercial merchandise (goods not considered personal effects)
- S. Residents: Total value for all goods acquired abroad
- For visitors/non-residents: Total value of all articles that will remain in the U.S.
- Signature – This is a way to verify that you have carefully read the information on the reverse side and made a valid declaration.
You should also indicate any items you must declare on the reverse side. Gifts should have a retail value.
CBP officers will decide on duty. The duty-free exemption of $800 for items brought with U.S. citizens is regular. Visitors are usually granted an exemption of $100. The current duty rate will apply to the first $1,000 over the exemption.
You must file a FinCEN 105 report if you have more than $10,000.
The entry of controlled substances, obscene articles, and toxic substances is generally forbidden.
