Description:
Form G-639 is suggested to be completed by non-U.S. citizens in order to request access to information under the Privacy and Freedom of Information Acts. Although any written format is accepted for request for a Freedom of Privacy Act or Information, the usage of Form G-639 is recommended. Failure to provide specific and complete information may lead to a delay or that the required records cannot be found. Completing Form G-639 with false statements may be subject to criminal penalties. Records being kept by the Federal agency are in general accessible to U.S. citizens and permanent residents as long as a request is made. Form G-639 (Freedom of Information/Privacy Act Request) may also be known as USCIS Form G-639, G 639, Form G639 or immigration form G-639.
