Statement of Nonreceipt of Passport

Description:
Form DS-86 may be filed by a U.S. citizen to report that the passport applied for was never received. There are several reasons why non-recipients should file Form DS-86 including protection of identity theft and prevention of misuse. Form DS-86 may be completed on a computer by using Adobe Reader before it is printed and signed. Form DS-86 may also be known as Department of State Passport Form DS86, DS 86, DS 0086, Form DS86 or immigration form DS-86.
