Statement Regarding Lost or Stolen Passport

Description:
Form DS-64 should be completed by applicants that are applying for a new passport because they have had their previous passport lost or stolen. Form DS-64 is a statement indicating why the previous passport cannot be submitted with the replacement/renewal application. The information provided on Form DS-64 is further recorded in the Consular Lost or Stolen Passport System. Individuals that later find their lost or misplaces passport must submit it for cancellation as it is invalidated and cannot be used to enter the United States. Form DS-64 (Statement Regarding Lost or Stolen Passport) may also be known as Department of State Passport Form DS64, DS 64, DS 0064, Form DS64 or immigration form DS-64.
