Description:
Form DS-1981 is an affidavit that exempts a foreign adopted child from different vaccination requirements. Form DS-1981 exempts a child 10 years or younger from the immunization requirements should the (prospective) adoptive parent (immediate relative) provide Form DS-1981 stating his/her awareness of the provisions and ensuring that the child will receive the vaccinations identified at the earliest time possible or within 30 days of the child’s admission. Form DS-1981 may also be known as Department of State Form DS1981, DS 1981, Form DS1981 or immigration form DS-1981.
