Description:
Form DS-11 is updated as of February 2, 2008 to include application for both the regular Passport and the new U.S. Passport Card.

Form DS-11 is used to apply for a U.S. Passport / U.S. Passport Card for first time applicants and for individuals who have had their passport stolen/lost. Individuals seeking to renew their passports may be eligible to use Form DS-82. Form DS-11 contains special instructions for children. You may complete Form DS-11 on your computer by using Adobe Reader before it is printed. Form DS-11 (Application for a U.S. Passport / U.S. Passport Card) may also be known as Department of State Passport Form DS11, DS 11, DS 0011, Form DS11 or immigration form DS-11.

Important Warning:
Form DS-11 is available for download for your convenience. However, it may not contain filing instructions. It is vital to submit the correct form for each particular case. If you submit an incorrect form, do not complete it correctly, or do not provide the necessary documentation, your application may be denied.
