Purpose of Form AR-11SR:
This form is used to report an individual’s change of address in the United States that is subject to Special Registration.

Description:
Form AR-11 SR must be completed by non-U.S. citizens subject to Special Registration requirements within 10 days of change of address. The consequences if convicted may be a $200 fine and/or imprisoned for up to 30 days or deportation from the United States. Form AR-11SR (Change of Address Form - Special Registration) may also be known as USCIS Form AR-11SR, AR 11 SR, AR11SR, Form AR-11SR or immigration form AR-11SR.
