Form N-648, Medical Certification for Disability Exceptions

Form N-648 may be filed by non-U.S. citizens applying to become a U.S citizen and seeking an exemption from civic requirements based on disability exceptions and medical certifications. Form N-648 must be filed together with Form N-400. Form N-648 must be completed by a licensed clinical psychologist or licensed medical doctor. Form N-648 may be filed for the USCIS to determine if the applicant is entitled to exceptions to the requirements. Form N-648 is used by USCIS to have applicants demonstrate their knowledge of the English language and the fundamental principles and form of U.S. government along with the history of the United States. An applicant that demonstrates the knowledge that is required by the USCIS but has impairment(s) and/or disability to learn may apply for a waiver.
