Form I-693, Medical Examination of Aliens Seeking Adjustment of Status

Form I-693 must be filed by individuals that apply for adjustment of status. Form I-693 can only be completed by a physician that is approved by the USCIS. All expenses related to the health check and completion of Form I-693 must be paid by the applicant. An adjustment of status application is not complete unless Form I-693 is included and approved by the physician.
