Form DS-71, Affidavit of Identifying Witness: Information Sheet

Form DS-71 is an affidavit used to identify a passport applicant in cases where the individual is not able to proof his or her own identity to an Authorized Acceptance Agent.

The form described in this information sheet is to be completed by the identifying witness, not the passport applicant. Form DS-71 is obtained from the passport agent, and must be signed in front of the agent and included with the passport application. Individuals that do not provide correct information may face consequences of fines and/or imprisonment. Form DS-71 may also be known as Department of State Passport Form DS71, DS 71, DS 0071, Form DS71 or immigration form DS-71.
