Form DS-158 must be completed by a non-U.S. resident that have submitted a visa application DS-156, and been requested to provide additional information pertaining to his/her work history and contact information. Visa applicants who have filed the new application form DS-160, do not need to file Form DS-158.

Form DS-158 can be completed on your computer by using Adobe Reader. Form DS-158 may also be known as Department of State Form DS158, DS 158 Form DS158 or immigration form DS-158.
