Form DS-157 is sometimes required as a supplement to U.S. visitor’s visa applications that are filed using the older Form DS-156. Visa applications that are filed using the new Form DS-160 do not require Form DS-157.

Form DS-157 (Supplemental Nonimmigrant Visa Application) may also be known as Department of State Form DS157, DS 157, Form DS157 or immigration form DS-157. Form DS-157 may be completed on your computer by using Adobe Reader.
